What You Didn't Know About Piles
A lot of myths abound Piles and its treatment. Call it Piles, Haemorrhoids, Mulavyadhi [Kannada], Baoseer [Hindi\Urdu], it basically amounts to any Ano-Rectal problem.
Common diseases affecting the ano-rectal region:
Piles; Fissure; Fistula; Prolapse; Proctitis; Cancer.
What is Piles?
Piles are normal sponge-like structure arranged in three groups, at the lower end of anal canal. They act like valves in the finely balanced ‘Continence’ mechanism (identification and regulation of passage of solid, liquid or air.) They are filled with blood vessels which:
a) when they enlarge and rupture, they bleed.
b) when they are displaced, they protrude outside and can cause severe pain, bleeding, discomfort and complications.
What are the factors responsible for intra-anal pressure?
Constipation; Straining during passing of stools /urine; Prolonged standing/sitting; Hereditary; Pregnancy; Over-purgation; Cancer of rectum.
What are the symptoms?
INITIAL: Painless bleeding while passing stools.
LATE: Discharge of fluid from anus with itching, heaviness\fullness in the rectum.
Feeling of incomplete evacuation.
Protrusions outside the anus that can be pushed back.
LATER: Painful protrusions- cannot be pushed back; Anemia.
Classification of Piles
They are classified depending on their position, in relation to Anus:
I degree Piles: Remain Internal.
II degree Piles: Protrude outside during defecation and go back on their own.
III degree Piles: Protrude outside during defecation and to be pushed back inside.
How to confirm
* Simple Rectal examination by Proctologist\Surgeon – A painless procedure.
* Video Rectoscopy – for more precise diagnosis.
Complications if not recognized early and treated properly:
1 Severe bleeding.
2 Severe painful swelling and infection of protruded masses.
3 Other causes of bleeding per rectum – like cancer, fissure etc., can be missed.
Why is a common man afraid of going to a surgeon?
* Ignorance of the nature of disease
* Ignorance of the recent treatment options available which are entirely safe, painless and scientific, which give permanent cure and do not require admission and major operation or absence from work.
* Disinformation: Every system of medicine and quacks claim cure for Piles.
What are the treatment options available in modern scientific system?
For bleeding Piles (internal)
1. Injections of chemicals.
2. Cryotherapy – over cooling
3. Rubberband application.
4. Infra Red Coagulation – recent method (Photo Coagulation) precisely controlled burning of blood vessels going to Piles, thereby reducing congestion inside the Piles.
5. Laser – Coagulation.
6. Radio Frequency ablation /Excision – Bloodless surgery for removal of piles, fistula, polyps, tumours, etc with least bleeding & with clear vision.
7. Stapled haemorrhoidopexy (MIPH).
8. DGHAL – Precision closure of vessels supplying Piles [Doppler Guided Hemorrhoidal Arterial Ligation].

Treatment for bleeding Piles: It is undertaken on an out-patient basis, the procedure is done within a couple of minutes and patient can attend to his work early.
Now, state-of-art technologies like Radio-Frequency ablation, Stapled haemorrhoidopexy and DGHAL, etc have made treatment of protruded piles, virtually pain free and recovery faster.
In a gist, all the anal problems are not (caused) by Piles; all that bleeds is not piles – can be cancer / fissure etc; all the piles – need not lead in bleeding; taking any form of treatment (pills, churans, ointments, injections, etc) without confirmation of the diagnosis can be catastrophic; Simple, safe, scientific and cost -effective treatment is available without fear of pain, hospitalization and prolonged rest.
